Sports Injury Fast Track Program

If you have a referral from your primary care physician, please book your AESM appointment (link to form), or call us at 416-800-0800.

Patients must be referred by a physician to receive an appointment via the fast track program. If your physician / family doctor has not yet sent this referral, simply have him or her download this AESM Sports Injury Referral Form, with the Fast Track box checked off, and fax it to 416-800-0802.

If you do not have a family physician, and are in the downtown core, please consider seeing one of our primary care physicians at:
Compass Medical Centre
200 Wellington St. W., next to Metro Hall
Toronto.

This is a walk-in clinic, where physicians can see you without an appointment, to quickly refer you regarding your acute injury.
